Recreation Centre Corporate Membership Request - Employee/Retiree

This form is for use when employees or retirees of the Municipality are requesting to purchase a membership for the Nature Fresh Farms Recreation Centre. Those eligible for a discounted memberships include:

• Current full-time employees
• Fire Services employees
• Spouse/Partner of current full-time employees with a membership 
• Retirees who retired from full-time service with the Municipality

Part-time employees and retirees of the Leamington Police Services and PUC are not eligible for corporate membership rates.
