Facility Reservation Request

For all requests including Nature Fresh Farms Recreation Centre Facilities, Baseball Diamonds, Pure Flavor® Soccer Complex , Picnic Shelter/Pavilion and Beach Volleyball Courts. 

Applicant Checklist

Please read the below requirements:

  1. The applicant understands that use of the Facility may include possible exposure to infectious diseases including but not limited to the communicable disease COVID-19 coronavirus disease(“COVID-19”).
  2. The applicant has required each member of its organization or party who will attend at the Facility eg. coaches/volunteers/members/participants/attendee (or in the case of a minor, a parent or guardian) to execute a waiver or release of liability releasing the applicant from any liability in connection with the operations to be conducted by the applicant at the Facility.
  3. The applicant agrees that the following indemnity provision included in the Terms and Conditions for the use of the Facility applies to any claims related to COVID-19:

Release and Indemnity: The Licensee releases and discharges The Corporation of the Municipality of Leamington, its elected or appointed officials, servants, agents and employees from all claims, demands, losses, costs, charges, actions, causes of action or liabilities in respect of any personal injury or death, or loss or damage to property which may be sustained during or arise from, directly or indirectly, the use of the Facility that is the subject of this Permit.  Further, the Licensee shall at all times indemnify and save harmless The Corporation of the Municipality of Leamington, its elected or appointed officials, servants, agents and employees from and against any and all claims, demands, losses, costs, charges, actions, causes of action, liabilities and other proceedings made or brought against or imposed on the Municipality its elected or appointed officials, servants, agents and employees in respect of any personal injury or death, or loss or damage to property which may be sustained during or arise from, directly or indirectly, the use of the Facility that is the subject of this Permit. 

4. The applicant undertakes and agrees that, while using the Facility, it shall comply with the following related to COVID-19:

  1. any regulation or order issued pursuant to the Emergency Management and Civil Protection Act and/or the Reopening Ontario Act, 2020; and
  2. the advice, recommendations and instructions of public health officials, including any advice, recommendations or instructions on physical distancing, cleaning or disinfecting.

5. The applicant has written policies and/or procedures to ensure its compliance with the following related to COVID-19:

  1. any regulation or order issued pursuant to the Emergency Management and Civil Protection Act and/or the Reopening Ontario Act, 2020; and
  2. the advice, recommendations and instructions of public health officials, including any advice, recommendations or instructions on physical distancing, cleaning or disinfecting.

6. The applicant has a safety plan as required by section 3.3 of Schedule 3 of Regulation 364/20 under the Reopening Ontario Act, 2020.

7. The applicant will ensure that each member of its organization or party who will attend at the Facility eg. coaches/volunteers/members/participants /spectators/attendees (or in the case of a minor, a parent or guardian) understand and follow the policies and/or procedures as referenced in section 5 and the safety plan as referenced in section 6 herein.

8. The applicant will produce the policies and/or procedures referenced in 5 and the safety plan as referenced in section 6 for inspection by the Municipality upon request.

Important:

Spectators are strongly discouraged. Family members or guardians should avoid the field unless they are picking up or dropping off players. Everyone visiting a sports field must stay at least two metres away from people outside their household or social circle.

All leagues and user groups are required to ensure coaches, volunteers, parents and participants understand and follow return-to-play guidelines.

 

Statement - Online Form Submission:

By submitting this online form to the Municipality, the user represents and warrants that the user has the authority to bind the applicant; acknowledges that the submission is deemed to be a substitute for the user’s signature on the online form and signifies that the user has read, understood and agreed to the contents within the online form and any document referenced in the online form and agrees that the applicant is bound by the content herein, including the Applicant Checklist.
