Information/Documentation to be attached to each Applicationf Form: 

Certificate of Insurance issued and signed by a licensed insurance broker for the province of Ontario or an agent or employee of the insurer. Insurance for each Taxicab shall be for liability not less than $2,000,000 per accident exclusive of interest and costs.

The Information requested on this application form and any appendendices attched are collected under the authority of the Municipl Act and By-law 63-16, and will be maintained in accordance with the Municipal Freedom of Information of Privacy Act. The information is required in order to process, dertmine eligibility for issuance of a Taxicab Licence, and to monitor, regulate and investigate licenses issued by the Municipality of Leamington.
