Application for Spay or Neuter Voucher

Note: Please note that on the date this application is submitted, you must have been a resident in the Municipality of Leamington for at least one (1) year. Qualifying applicants are allowed a maximum of three (3) Spay or Neuter Vouchers for Low Income Families and a maximum of three (3) Spay or Neuter Vouchers for Feral Cats per family per year.

Pick-up Appointment: The Municipality of Leamington is committed to protecting your personal information and will not accept copies of identification or other supplementary documents for this Application electronically, by mail or drop box. Upon receipt of this Application, a Customer Service Representative will phone you at the number you have provided to schedule a voucher pick-up appointment. At your scheduled appointment, you will provide copies of any outstanding documents. Further instructions will be provided by a Customer Service Representative.

The information I have provided is true and complete to the best of my knowledge. I am at least 18 years of age, have been a resident of the Municipality of Leamington for at least one year and my residence in the Municipality of Leamington is my primary residence. I understand that if I do not sign this application, the application will not be processed. 

I understand that vouchers are issued on a first come, first serve basis and that they will expire within 90 days from the date of issue by the Municipality of Leamington. If I am unable to use the voucher(s) issued to me by the Municipality within 90 days of the issue date, I agree to return the voucher to the Municipality.

The information will be used for the purpose of processing applications for Spay or Neuter Vouchers for Low Income Families and Spay or Neuter Vouchers for Feral Cats.
